Purpose
Are you a Physical Therapist looking for a rewarding career providing direct, one-on-one patient care? We are seeking caring and skilled rehabilitation therapists to join our Home Healthcare team. You will work in a home setting with a diverse patient population, guiding their care in the comfort of their own environment. You will collaborate with a specialized team of nurses, social workers, and dieticians to provide truly personalized patient care.
Responsibilities
- Clinical Care: Evaluate, plan, and treat patients using an effective physical therapy program, including diagnostic, muscle, nerve, and functional ability testing.
- Documentation: Maintain and complete all documentation promptly and accurately (including OASIS, 485, and Verbal Orders) on a laptop device during the home visit.
- Collaboration: Establish plans of care in collaboration with physicians and the interdisciplinary team. Advise and consult with family members and other professionals to ensure optimal care.
- Supervision: Assign and supervise Physical Therapy Assistants according to State Board guidelines.
- Professional Development: Continually develop professional skills and participate in the agency's on-call coverage rotation.
- Operational Excellence: Data sync (transmit) clinical information the same day and participate in process improvement opportunities to enhance service quality.
Minimum Essential Requirements
- Graduate of an accredited Physical Therapy curriculum approved by the APTA or equivalent governing body.
- Current Physical Therapist (PT) licensure.
- Minimum of one (1) year of clinical experience to practice without supervision (or less than one year if practicing under supervision in specific settings like ALF/PCH).
- Current Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.
- Valid Driver’s License and Automotive Insurance.
- Ability to clear all required background checks (Act 33, Act 34, OAPSA).
